چکیده

Abstract Background Several clinical data in humans demonstrate that atrial fibrillation (AF) dominant frequencies and rotors are mostly localized on the posterior wall (PW), but correlation among these phenomena abnormal conduction properties during sinus (SR) paced rhythm (PR) has never been analyzed. Aims. To provide a characterization of left PW patients affected by persistent AF. In particular, purpose is to describe electrophysiological terms rotational focal activities AF, propagation uniformity SR functional sites definition PR. Methods Six from two EP centers have selected for this analysis. The procedures performed with CARTO3 mapping system. All EGMs collected Pentaray Octaray catheters. For each patient, module CARTOFINDER used AF identify areas shorter longer cycle length (site jump), activities; at least 15 PW. PR map were after electrical cardioversion exploit phenomena. created setting an extra beat coronary every three beats (pacing interval=local effective refractory period +40ms). Rotational identified associated signals characterized voltage amplitude (mV), signal duration (ms) fractionation. Results Abnormal recorded 33; one site jump was found most cases (77%, 10 out 13) it which 7 (70%) activity 2 (in PW). During rhythm, interest, we lower voltages (SR 1.9 ± 1,3mV, 1,7 1,15mV), fragmentation 52,7 8,85ms, 51,3 11,04ms) compared healthy tissue. Conclusion There strong between interest abnormalities This research work may pave way study represent relevant element maintenance, providing further step towards substrate

The Atrial Fibrillation Substrate: Pulmonary Veins, Posterior Wall, or Both

Atrial fibrillation (AF) is the most common arrhythmia seen in clinical practice and is associated with a notable increase in morbidity and mortality. Although there have been considerable advances in the treatment of AF, the results of drug treatment and ablation remain suboptimal.1 This is largely due to the fact that there is still little understanding of the mechanisms responsible for the i...

Complete Isolation of the Left Atrial Posterior Wall (Box Lesion) to Treat Longstanding Persistent Atrial Fibrillation.

INTRODUCTION The left atrial posterior wall has been shown to play na important role in AF pathophysiology. OBJECTIVE Evaluate the efficacy of an ablation strategy designed to completely isolate the LA posterior wall, on top of PV isolation. Methods and Results: 25 pts (72% male age 65 ± 12 years) undergoing AF ablation for persistent or long term persistent AF. Mean AF duration was 11 ± 3 mo...

of the right lower lobe, draining in the posterior aspect of the LA. The 4 PVs were electrically isolated by radiofrequency ablation, while sparing the accessory PV because of its small diameter and absence of electric activity in the interior. Accessory or supernumerary PVs can arise during embryologic development, and result in additional, separate openings of the LA with typically narrower a...
